B. A COMPLETED SHIPPING ORDER

Also, please ensure the following information is submitted to us (Photocopies only, please):

A. Your Passport or Birth Certificate.
B. A Copy of the Vehicle Registration.
C. A Copy of your Insurance Card.
D. A Letter of Intent
